New Patients

To schedule an initial appointment for any of our services call the main number at 980-230-1263 (8:00 am – 7:00 pm; M-F) and ask for the Intake Department. One of our Intake Coordinators will speak with you to get the information needed, schedule a convenient time and set up transportation if needed. 

For Referrals

Please call and ask for one of our Intake Coordinators to make a referral for a patient, family member or yourself to receive services at TEAMS, LLC. Basic information including telephone number, address, social security number, date of birth and the Medicaid / private insurance number is helpful. 

Established Clients

To contact the TEAMS, LLC provider that works directly with you, call TEAMS, LLC main line at 980-230-1263 during business hours. If the need is urgent, there is always someone available to speak with you.

After-Hours Emergent Situations

For current service recipients with urgent needs, call the appropriate TEAMS, LLC after-hours crisis line number listed on your treatment plan. If you need immediate mental health support and crisis counseling, reach out to 988. If physical health or safety is at risk, dial 911 immediately.
